Medicare Advantage plans generally have coverage for home health services but may limit your post-acute care severely in terms of coverage compared to the traditional benefit and in terms of choice of provider. Your choices will usually not be based on which providers have the highest quality of care or satisfaction scores. Check your home health benefit and other referral-based care coverage before opting in to a Medicare Advantage plan.

Homebound: Most importantly, patients should meet Medicare's definition of homebound. This does not mean a patient can't leave the home. Homebound means that patient's need equipment (such as canes or walkers) or human assistance to leave the home safely; that leaving the home requires a taxing effort or is against medical advice; and that trips away from the home are infrequent and of short duration. Patients may be temporarily homebound due to injury or illness.

Physician Supervision: Home health patients must be under the care of a doctor (MD, DO, or DPM) for the home health diagnosis.
